hmm, *hands him a stack of paper pileed about a foot high. fill that out to get the second form, then fill out the scound form to get form a-1. after you fill that out you get form b-5 and a-12. fill those out to get a card that will allow you to set up an appointment. we will get back to you in 5-6 months with your appointment date.
